WHAT SERVICES DO YOU PROVIDE?
Primarily, we provide one on one nursing care in private homes in the greater Cape Town area. Our care assists with the Frail and Elderly, Post-op, Dementia Palliative and End of Life patients. We also provide live-in care. All care is under the supervision of our Registered Nurses. We also arrange for, and co-ordinate the home visits of Physiotherapists and Doctors,

HFN services the greater Cape Town area from Simon's Town in the south, Kraaifontein in the north, Parklands in the west as well as the Atlantic seaboard and Southern suburbs
I'VE CHOSEN HFN - NOW WHAT?
A Home Assessment with our Registered Nurse is scheduled
HFN will then make a recommendation and quote accordingly
If relevant, a quotation plus Doctor's motivation for home care is sent to Medical Aid for authorization
HFN Contract is signed and deposit paid.
Comprehensive Care Plan is drawn up
Appropriate Care giver is assigned and introduced to patient by HFN Case Manager
IS THE PRICE ALL INCLUSIVE OR DO I NEED TO PAY ANYTHING ELSE?
The rate is all-inclusive. There are no hidden costs, admin fees, joining fees, monthly or annual fees
IS THERE A DEPOSIT PAYABLE?
Yes - we require a deposit of 50% of the patient's monthly billing on commencement of service. The deposit is fully refundable on termination of service
IS THERE A CONTRACT TO BE SIGNED?
Yes - a contract setting out the terms and conditions of service, the care givers duties and responsibilities and scope of practice must be signed on commencement of service
HOW AM I INVOICED, AND HOW DO I PAY?
Invoicing is done every two weeks. Invoice is e-mailed to person responsible for payment and is due on presentation. Preferable method of payment is via EFT
WHAT ARE THE DUTIES OF HFN'S REGISTERED NURSE?
To conduct initial home assessment
To compile a Patient Care Plan
Ensure Care Giver is carrying out the care plan
Update Care Plan as and when required
Regularly monitor the patient's health, and well-being as well as the Care Giver's conduct
Compile regular reports on the patient's health and Care Giver's conduct
WHY IS A HOME ASSESSMENT IMPORTANT?
An assessment enables us to evaluate the patient's physical, mental and emotional needs so we can offer the best care
Combining this information with the doctor's medical report and input from the family, a comprehensive Care Plan is drawn up
