Stepdown Facility vs HFN’s Stepdown@home

A conventional Stepdown Facility is anywhere from R2,800.00 to R4,000.00/day

HFN Stepdown@home: 24/7 Home Nursing for R1,913.60 per day.

Saving of up to 40% over a stepdown facility.

HFN will scale down care as the patient recovers, thus increasing the savings.

A comprehensive Home Nursing Care Plan

One-on-one care from ENA 24/7

Hands-on RN supervision.

Referrals to any of the multi-disciplinary team members.

Hiring/Purchasing of Medical Equipment - hospital beds, oxygen therapy, wheelchair, crutches, etc.

 

The Multi-Disciplinary Team:

HFN’s Registered Nurse – for overall supervision and consultation

Physiotherapist – we refer mostly to Hector, Naidoo and Associates. Physio@Home

Occupational Therapist – OT@Home, Clarice Roode.

Speech Therapy– Speechtherapy@Home among others

Wound Care, Stoma etc. – WoundNet.

The Multi-disciplinary team use the availability of the 24-hour/one-on-one nursing to instruct the Nurse/family in care responsibilities after the therapy sessions.

 

Other support services available:

Social Worker: In cases where the family surrounding the patient is dysfunctional referrals to psychiatrist and/or curators.

Home visiting doctor - Kimoramed.

Dietician – Mindful Eating- Mignon Jordaan

Medication blister packaging – Vitacare Pharmacies

Medical equipment - Wheelchairs, hospital beds and pressure mattress, bedding packs and drawsheets, oxygen therapy, bath-lifts, hoist, etc.

Nappies – adult nappies delivered to the home

Tailored Comprehensive Home Care Plan

The HFN Care Plan provides comprehensive tailored, individual 24/7 care. Home Care can be reduced as the patient condition improves. Our Multidisciplinary Team will address and support all patient needs ensures that all healing needs are addressed to support optimal outcomes. HFN is thus able to provide individualized home nursing for optimal recovery.

 

 

Compassionate Care

HFN offers palliative and respite care. Nurses and Carers will improve the quality of life of the termly ill, ensuring as pain free, dignified, and comfortable life as possible. HFN Respite home care services offer short term relief for the caregiver/ family.

 

Medical Services

The HFN Registered Nurse will assess the patient and refer him or her to the relevant HFN Multi-Disciplinary Team (MDT) members who are able to assist with:

Post-op Orthopaedic conditions

Palliative care

Oncology care

NGT and PEG feeding

Stoma care

Wound care

Stroke, heart attack rehabilitation

 

 

 

The Stepdown@home Process

 

HFN offers the whole slate package when providing the Stepdown@Home service.

Step 1

Doctor arranges the theatre, hospital stay, post-operative care and gets authorisation from patient’s Medical Aid. HFN Nursing Agency and other health professionals will submit quotations for two week’s step-down care at home to the Doctor for submission to Medical Aid. HFN obtains authorisation and deals directly with patient’s case manager.

 

Step 2

On  discharge, script and transport is arranged by the Ward Sr. (HFN can arrange for private ambulance transfer should Medical Aid not pay for this.)

The Doctor provides discharge instructions for our RN to execute at patient’s home. The RN sets up a care plan for the Home Nurse to follow.  HFN will arrange the following health professionals - PT, OT, Wound care, Dietician, etc, as per the Doctor’s requirements.

 

Step 3

The home care nurse will assist the patient home or meet at the patient at home and begin nursing as instructed. The Nurse will familiarise herself with patient’s family and house routine. HFN’s RN does first visit and then daily visits. Other health professionals requested will do their introductions and visits as prescribed. They will instruct the Nurse and family on how to help the patient to complete healing and recovery as well as help with sourcing mobility devices as required.

 

Step 4

HFN’s RN will keep in contact with doctor, and all other health professionals involved, on patient progress via the Signapps communication platform. This can include reports from all health professionals involved.

The HFN RN will do a Progress Report for Medical Aid should the patient needs longer care at home than the initial two weeks.

or

Conclude the nursing care and send a report to the doctor and then close the case.

Other health professionals will report to HFN when their services are concluded.
