Compiling an HFN Nursing Agency Care Plan

In compiling an HFN Care Plan the Registered Nurse will:

·       Assess the situation and evaluate the needs and capabilities of the patient.

·       Set goals – decide what care is needed and set realistic goals for the Carer and involve the family in decision making.

·       Determine what resources are available, such as support from family and friends, and community organizations.

·       Make a schedule for providing care, accounting for the needs of the patient.

·       Review and adjust the plan to meet the changing needs of the patient. Communicate this information with loved ones, keeping the family informed about the care plan and involve them in the care plan.
